Extra care should be exercised when conducting drills involving lifeboats lowered by falls, particularly in the following areas:

  1. Failure of the on-load release mechanism.
  2. Accidental operation of the on-load release mechanism.
  3. Inadequate maintenance of lifeboats, davits, falls, and launching equipment.
  4. Communication failures between crew members.
  5. Insufficient familiarity with lifeboats, davits, operating controls, and associated equipment.
  6. Unsafe practices during lifeboat drills and routine inspections.
  7. Design defects unrelated to the on-load release mechanism.

Careful planning, proper supervision, and strict adherence to established procedures significantly reduce the likelihood of accidents during training exercises.
